At the request of Dean Torjesen, the Religion Student Council is conducting this survey in an effort to foster more productive advisor/advisee relationships within the School of Religion. Please take a few minutes to support yourself and your fellow students by voicing your opinion on this important issue.

Please be confident that your name will not appear anywhere on this form and your personal information will be kept strictly confidential. We require your program information only for statistical purposes.

Please answer the following questions by selecting a number between 1-5, 1 for "not important at all," and five for "very important."

1. How important is the regular and consistent accessibility of your advisor?

2. How important is your advisor's timely response to your attempts to contact him/her?

3. How important are regular face-to-face meetings between you and your advisor? (as opposed to via email or telephone?)

4. How important is your advisor's familiarity with the specifics of your work/research interests?

5. How important is your advisor's ability to answer your academic/professional questions?

Section II: Knowledge
